To be considered for admission to the online Master of Science in Health Information Management program, you must submit all of the following items. Please follow each step in the application process carefully, as errors may delay the processing of your application. Once your application materials are received and reviewed, your admissions advisor will notify you of any additional information that is required.
1. Submit UIC's Graduate Application OnlineVisit the UIC Graduate Application page. Once on the page, you can either search for the program or select the Master’s Programs link under the program search bar. Select, or search for, Health Information Management – MS from the list of available programs. After you have selected Health Information Management – MS, click on Apply and create a log in.
You will be able to complete Steps 2 – 7 (listed below) within the online application portal.
Complete and submit the application along with the $70 non-refundable application fee.
2. Upload Unofficial TranscriptsYou will upload official transcripts from all schools you attended (can be electronic). Once you are accepted, you will be required to send official transcripts to our Graduate Admissions Office.
3. International Credential Evaluation FeeIf you are an international applicant or have taken foreign coursework, please refer to the foreign credential guidelines on the International Admission Requirements webpage. An international credential evaluation fee of $100 is assessed to any applicant presenting coursework taken outside the United States. The evaluation fee must be paid after submission of the application.
4. Personal Recommendation Letters (2 Required)Fill out the form, within the application portal, to have the graduate college send out email requests for 2 letters of recommendation (they must be current or former manager or supervisor and/or professor(s)). You will need to input the recommenders email addresses.
Suggested questions to forward to your recommenders, which they can address in their letters of recommendation:
Be sure to upload your most recent Resume, or CV, that details educational background, work experience, and honors/awards/activities, if applicable.
6. Upload Your Personal StatementAnswer the seven questions found in the Personal Statement section and upload the file into the application.

TOEFL scores must be submitted directly from the Educational Testing Service to the UIC Office of Academic & Enrollment Services using institutional code 1851.
